PROGAM ELIGIBILITY

The Birth to 3 Program is for children who are ages birth to thirty six months of age. Eligibility is based on a diagnosed disability or a significant delay in one or more areas of development. Along with staff the family will evaluate the child's ability to:

  • Play and Learn (Cognitive development)
  • Move, see, and hear (Motor/physical development)
  • Communicate and understand other's communication (Speech and language development)
  • Respond to and relate with people and objects in environment (social and emotional development)
  • Eat, dress, and care for daily living needs (adaptive development)

ENROLLMENT & SERVICES

After a child meets eligibility requirements, they may be enrolled to receive services from the Waupaca County Birth to Three Early Intervention Program. A service coordinator works with the family to ensure that the family receives services and supports needed within the family's natural environment.

The team (family members and Early Intervention Staff members):

  • Discusses the child's developmental strengths
  • Reviews families daily routines and activities
  • Determines what services and supports would enhance the child's development
  • Prepares a plan to provide services and supports within the families daily routines and activities.
  • Work as a team led by a primary coach to achieve the families goals they have established for their child themselves.

COST

  • There is no cost to you until the child has been found eligible for services and the family decides to enroll in the program.
  • All families who accept services will complete a cost share worksheet to identify the family's contribution.
  • With the family's permission, we will bill insurance for therapies provided.

Screening is provided for any child who is suspected of having a delay in development at no cost to the family.

A team made up of birth to three staff, parents, and other care givers will determine eligibility.

Through a team approach the early intervention program works to support and build upon what families are already doing on a daily basis.
